Minors (under 18) must have parent/guardian present
Vaccine Offered - Moderna & Pfizer
Patients will need to present their vaccination card with record of receiving previous dose(s), vaccine administration date and lot number, if applicable.
Consent & Attestation Forms
Copies attached for reference only will be signed electronically at clinic.
We can vaccinate anyone 12 years and older at our mobile clinics, in accordance to current COVID-19 vaccine guidelines Vaccine Cards
Patients are expected to bring their vaccination cards to the clinic so our team can document vaccines administered
If vaccine card has been lost, please obtain record through the following methods:
· Obtain via Hartford HealthCare’s MyChart Plus patient portal. Log into MyChart Plus.

· If you have an account, log into VAMS and view or download your Certificate of COVID-19 Vaccination or
· Call (860) 509-7929 to request your Official Immunization Certificate. from the CT Immunization Information System (CT WiZ) at the Department of Public Health.
